We have a great opportunity for a Registered Nurse with experience in CVD and diabetes to join our Long Term Conditions team.
Your focus will be improving diabetes and CVD care in primary health, and you will work with nurses and other practice staff. Key activities are the co-ordination and delivery of diabetes and CVD education for practice nurses and providing support to practices to provide excellent diabetes and CVD care. You may also assist in designing courses for other health professionals and/or patients.
You join a small friendly team dedicated to CVD and diabetes (registered nurses, dietitians and administrative support) who provide support and cover for each other where required.
The role requires sound New Zealand diabetes and CVD knowledge and experience which could be from primary or secondary (or both) health care experience. This could be an ideal role for a practice nurse wanting to extend their experience and expertise. This is a full time role, and is not suitable for new graduates or for nurses who have completed their CAP training after 2016.
You need to have excellent communication and presentation skills, and the ability to explain clinical matters to a non-clinical audience. Adult education experience is an advantage. You should have experience with one or more practice management systems, and experience with the Dr Info audit tool would be helpful. You must have a clean driver licence.
